Report of Birth, Marriage and Death.
It is necessary to notify the city office of any births or deaths.
Reporting of marriage between two foreigners is also possible.
(1) Birth
- When a child is born, one is required to report within 14 days from
the birth date. Note that the 14 days are inclusive of the birth date.

- Maternal and child health handbook.
- National health insurance certificate (only those insured).
- Others.
- Please notify your country’s embassy or consulate of the birth.
- If a child is to stay 60 days or more in Japan after his/her birth,
it is necessary to carry out the following procedures:
- Alien registration.
- Acquisition of residence qualification
(2) Marriage
<Documents necessary for reporting a Marriage>

Report format (Get signatures/ signets from witnesses who are aged
20 or above)
- Certificate of legal capacity to contract marriage issued by your country’s
embassy or consulate.
- Japanese translation of the above named certificate including the name,
address and signet of the translator at the bottom end of the translation.
- Certificate of the fact of your alien registration.
- Passport
* Please notify your country’s embassy or consulate of the marriage.
* Please contact us in advance, as necessary documents may vary by country.
(3) Death.
- A family member or a person who lived with the dead person need to
report the death within 7 days of knowing about the death.

- National health insurance certificate (for those who were insured).
- Others.
- Return the certificate of alien registration and national health insurance
certificate to the city office or respective branch office.
- It is necessary to acquire a permit for cremation, if the dead is
to be cremated.
- Please notify your country’s embassy or consulate of the death.
